What documents can be used as proof of residency in Arizona?

In order to get your Arizona driver’s license, you’ll need to provide proof of residency, and to do that you’ll need two different forms of proof. The
Arizona Department of Transportation
(ADOT) states that the two forms of proof must be from the mail.
Mail that is eligible for providing proof of residency needs to be issued from a business, organization, or official government body. Personal mail doesn’t count—you could bring in a whole sack full of birthday cards from your grandma, but they won’t do the trick!
The mail also needs to have your full name and your physical residential address listed. Mail received at a P.O. Box is not acceptable. 
Here are a few kinds of mail that can be used to provide proof of residency: 
  • Current bank statements
  • Current credit card statements
  • Current insurance policies
  • Official mail from a government body: city, state, or federal level
  • Current utility bills
  • Health insurance statements
  • Pay stub
  • Pension or retirement statement
  • Medical bills
  • Social security statement
This is in no way an exhaustive list. If in doubt, ask ahead of time to make sure your documents are acceptable.
All of your documents must be in English, and they must be originals or certified copies. Don’t forget to bring two completely different forms of proof! 

What documents do not qualify as proof of Arizona residency?

But remember, just because it has your name and address on it doesn’t automatically mean that it counts as proof of residency. Here are a few examples of some mail and documents that don’t count: 
  • Business cards
  • Club or organization membership cards
  • College or university ID cards
  • Fishing or hunting licenses
  • Insurance cards
  • Library cards
  • Personal mail 
  • Retail rewards cards

What qualifies for proof of identity in Arizona?

Besides proving that you live in Arizona, you’ll also need to provide proof of who you are. Is anyone else starting to feel like they’re having an existential crisis? 
At any rate, this involves some official documentation as well. To prove your identity, you’ll need to provide one of the following:
  • Birth Certificate—Original or certified copy of a birth Certificate (including “Government Use Only” birth certificates) issued by any state, territory, or possession of the US (Hospital records or certificates and California Certified Abstracts of Birth are not acceptable) 
  • US Certificate of Birth Abroad (FS-240,545 or DS-1350)  
  • Unexpired US Passport or US Passport card  
  • Permanent Resident Card/Resident Alien Card, I-551  
  • Unexpired USCIS Employment Authorization Document  
  • I-94 form with an unexpired foreign passport and an unexpired US Visa (excluding Visa Waiver program participants) 
  • US Certificate of Naturalization 
  • US Certificate of Citizenship
These documents also must be original and in English. They also need to have your name and date of birth on them. 
If you’ve changed your name and it’s different from what appears on the document that you’re using from the above list, then you’ll need to show legal proof of your name change (which needs to have been done with the Social Security Administration).

  • Divorce Decree (Restored Last Name)
