Replacing a lost Arizona license online

If you want to replace your lost Arizona from the comfort of your own home, you can apply for a duplicate
online
through the AZ MVD replacement portal. 
You will first need to verify your identity through one of two documentation options: 
  • Driver’s license number, birth date, and last four digits of your Social Security number
  • Social Security number, birth date, and last name
Once you verify your information, be prepared to explain why you need a duplicate license, and pay the $12 duplicate license fee. Be sure to print out your temporary license and carry it with you while you wait for your duplicate license in the mail. 
Your duplicate license should arrive within 15 days at the address on file. If you need your license sooner, however, you can opt to pay for express delivery.

Replacing a lost Arizona license in person

If you’re having trouble navigating the website or you need to update information (such as name changes due to marriage or court order), you can apply for a duplicate license in person at your
local AZ MVD
office. 
You will need to bring along documentation that proves your identity such as your birth certificate, US Passport, or a US Certificate of Naturalization. You will also need to present two documents verifying your address to establish residency in Arizona. These two documents must display your name and physical residential address. 
To be accepted, documents must be either original or certified copies by the issuing agency. You may use the

The fee to process the duplicate license is $12, which you may pay with either cash, a personal check or money order, or via credit or debit card. What if you lose your Arizona license out of state?

To replace your license by mail, write down all relevant information that you would need for getting a new driver’s license, including: 
  • Name and birthdate
  • Social Security number
  • Permanent Arizona address
