Then, you will need to provide two official documents that prove your residential address.
Qualifying documents
can be a utility bill, a pay stub, or a bank statement. The documents have to be original and cannot be photocopies.
You will also need to be prepared to pay a $15 replacement fee and a $2 payment processing fee.
