It’ll be easier to explain how you lost your ID, and your replacement should arrive faster. You will need to provide the following information to your local DOR office:
Your driver’s license number
Your full name
Your date of birth
Your residential address and/or mailing address
Proof of identity, like a birth certificate
Proof of Social Security number, like a social security card
