No matter why you need to replace your
Colorado driver’s license
, you can easily request a duplicate from the DMV in person for $12. If you’re out of state, you can request a replacement license by mail.
  • You must go to the DMV in person to request a duplicate driver’s license.
  • If you’re out of state, you can mail a duplicate request form to the DMV.
  • Show proof of identification and pay the $12 fee to get a duplicate license.

Get a duplicate license in person at the DMV

To request a duplicate
Colorado
driver’s license or identification card, you must visit a driver's license office in person. Unlike other states, you are not able to request a replacement online.
If it’s only been 30 to 90 days since your license was issued, you won’t have to pay any fees—otherwise, you’ll have to pay a $12 duplicate fee.
Schedule an appointment at your local Colorado DMV office using the Colorado DMV’s
appointment scheduling portal
. Choose “License, ID renewals” when you’re designating your appointment type.
Once you’re at your appointment, you’ll have to declare your license as lost, stolen, or destroyed to be approved for a duplicate credential. Note that you are not
renewing your Colorado license
—your duplicate will have the same expiration date, class, and endorsements.
You’ll also have to bring documents that show proof of identity and
proof of Colorado residency
. A valid U.S. passport, birth certificate, or certificate of naturalization are all examples of acceptable forms of identification. Proof of residence includes:
  • First class mail
  • Credit card statement
  • Lease agreement or mortgage statement
  • Bank statement
  • Pay stub
  • Utility bill
  • Vehicle registration or title
You should receive your duplicate license within 30 days of your appointment. If you don’t, check the status of your replacement driver’s license on
myDMV.Colorado.gov
.

Report a lost or stolen driver’s license

If you’re dealing with a lost driver’s license or a stolen Colorado driver’s license, your personal information is compromised. Once you request a duplicate, report the card as stolen to your local law enforcement agency. Don’t forget to get a copy of the police report.
You can also notify your creditors and close any accounts that might be compromised if your license was stolen.
Pro tip: If you’re concerned about identity theft, you can request a new license (as opposed to a duplicate). First, you must file a report for local law enforcement. Then, take the police report with a notarized
affidavit of identity theft
to your driver’s license office
appointment
.

Request a duplicate by mail if you’re out of state

Lost your license while you’re not in Colorado? No worries! Since you won’t be able to request a duplicate in person, you can submit a request for a duplicate driver’s license by mail. Just make sure to double-check your duplicate fee
online
before you send in your request. As of late 2023, the fee is $12.
All you have to do is fill out a request form
Form DR 2989
(make sure to use the address where you want your duplicate sent), and mail it with your duplicate fee (cash or check) to the following address:
Colorado Department of Revenue
Colorado Division of Motor Vehicles
P.O. Box 173345
Denver, CO 80217-3345

FAQs

Take documentation to your appointment that shows proof of identification. For instance, you could use a valid U.S. passport, birth certificate, or military ID card, along with a valid mail like a utility bill, credit card statement, or housing payment.
The state of Colorado charges a $12 fee to issue a duplicate license. However, if you’ve already gotten a duplicate, the fee goes up to $16.
