How to replace a lost driver’s license online in Indiana

To replace your lost Indiana driver’s license online: You’ll have to create a
myBMV
account if you don't already have one. Here’s what you’ll need:
  • Your driver’s license number
  • The last 4 digits of your Social Security Number
  • Your legal ZIP code
You’ll need to meet the following conditions to replace your license online:
  • Valid driver’s license
  • U.S. citizen
  • No Restriction J or a Restriction 2, 3, 4, 5, 6, 7, 8, or 9 on your driving record
  • Photo on file

Replacing a lost Indiana license in person

If any of the following conditions apply, you may need to replace your lost or stolen Indiana driver’s license at your
local BMV office
:
  • Your residential address, zip code, or township has changed
  • You changed the information on your driver’s license (such as a name change) with the BMV after receiving your last card
  • You need to change the address on your driver’s license
If you’re going to get a replacement driver’s license in person, remember: Only originals of the required documents will be accepted when proving your identity and Indiana residency, including for your legal name, date of birth, and Social Security Number. Examples of original documents for proof of identity include a Social Security Card or birth certificate.

What if you lose your Indiana license out of state?

If you lose your Indiana driver’s license while you’re out of state, you can get a temporary 30-day replacement by filling out the
request form
Once completed, you’ll need to mail the form to the following address:

If you recently renewed your license online: You may be able to print an interim license from your myBMV account. 
If you are out of Indiana due to being a member of the military: Your license remains valid for 90 days following discharge. You may also be eligible to renew or get a license replacement online.
