A physician, physician assistant, advanced registered nurse practitioner (ARNP), osteopathic physician, chiropractor, podiatrist, or optometrist will have to sign your application certifying that you meet one of the above requirements.
A temporary disability will qualify you for a placard that is valid for up to six months, while a permanent disability will qualify you for a 5-year placard. Once you have a 5-year placard, you have the option of applying for a license plate with a disability tag.
