You have a few options when it comes to replacing your lost Connecticut license—you can request your replacement online, in person, or by mail.

Replacing a lost Connecticut driver’s license online

Requesting a replacement Connecticut license
online can take as little as five minutes from the comfort of your home, but you’ll have to wait for your replacement to come in the mail. All you need is your driver’s license number, Social Security number, and a debit or credit card.
However, not everyone can request a replacement license online. You’ll have to go in person to replace your license if you fall into one of the following circumstances:
  • Your name has changed since the license you lost was issued
  • You carry a “Drive Only” license
  • You carry a Commercial Driver’s License

Replacing a lost Connecticut driver’s license in-person

Make an appointment
at your local DMV office or visit a
DMV Express office
to request a duplicate Connecticut license in person. You can fill out an
Application for a Duplicate License
(Form 1-B) at home and bring it to your appointment with a valid form of payment.
If you choose to visit an express office, you’ll have to pay an additional $8 fee on top of your specific license replacement fee. DMV Express offices include the following locations:
  • AAA offices
  • Nutmeg State Financial Credit Union offices in Milford and North Haven
  • West Haven City Hall
Drivers carrying the following types of licenses are not permitted to request a replacement at an express office, and should make an appointment at a regular DMV office instead: 
  • Commercial Driver’s Licenses
  • Commercial learner’s permits
  • Licenses with passenger endorsements
  • “Drive only” licenses
Once your application is approved and your fees are paid, you’ll receive a temporary paper license until your duplicate replacement comes in the mail.

Replacing a lost Connecticut driver’s license by mail

You can request a replacement Connecticut license by post or e-mail so long as you don’t fall into one of the following categories:
  • Your name has changed since the license you lost was issued
  • You carry a “Drive Only” license
If you choose to request your replacement by mail, you’ll need to fill out a
Request for a Duplicate License by Mail
(Form B-350). Make sure to check the
applicable fees
based on the type of license you are replacing.
To submit your request by post, you can send the form and fee (cash or check) to the following address:

Attn: Centralized Issuance Operations Unit
To submit your request by e-mail, you can send just the form to
dmv.ciu@ct.gov
. Once you’ve e-mailed your form B-350, you’ll be sent a payment link within two business days to submit your payment and finalize your request.
Once your form and fee have been received, you should receive your duplicate by mail in about five to seven business days.

What if you lose your Connecticut license out of state?

If you lost your license while you’re outside of Connecticut, don’t fret. You can still follow the steps to replace your license online or by mail if you meet one of the following criteria:
  • You’re an active-duty military member
  • You’re a prison inmate
  • You are only temporarily out-of-state
  • You have health issues that prevent you from appearing in person
If a medical condition is involved in your request to obtain a duplicate license, you will have to fill out Form CI-1 to include in your application. 
Your license will be sent to the address you include on your application, so make sure to provide the address where you want to receive your license.
