If you lose your Hawaii license out of state, you will need to write a letter with the following information:
  • The name on your Hawaii driver's license
  • Your Social Security number (SSN) or Hawaii driver's license number
  • Your date of birth
  • The telephone number to reach you
  • Your current out-of-state address

  • Your signature
