What you’ll need to get a replacement title in New Jersey

To request a duplicate title in New Jersey, you’ll need the following documents and information: 
  • A completed
    Universal Title Application
    (Form OS/SS-UTA)
  • The original title (if damaged)
  • Current or expired registration or
    proof of insurance
  • A $60 fee paid by:
  • Check or money order (if you’re applying by mail)
  • Credit card or cash (if you’re applying in person)
  • Signed lien release statement (if the vehicle is leased or financed)
  • In this case, the duplicate title will be issued by mail to the lienholder
  • If you don’t have proof of ownership, you will also need the following:
    • A completed
      Title/Lien Search Request
      (Form DO-22) (if the vehicle has never been registered in New Jersey)
    • A completed
      Vehicle Registration Application Request
      (Form DO-11A) (if the vehicle has previously been registered in New Jersey)
    • A $15 check or money order made out to “NJMVC”
    • Valid photo identification
    • Two color photographs of the vehicle from the front and the back

    How to apply for a replacement title in New Jersey

    The easiest way to apply for a replacement or duplicate title in New Jersey is in person. Use the
    Motor Vehicle Commission’s online portal
    to schedule an appointment at your nearest Vehicle Center. Bring your completed title application, proof of insurance or registration, and a $60 fee (payable by credit card, check, money order, or cash).

    If you don’t have proof of ownership, you must request your replacement title by mail
    The first step is to complete either the
    Title/Lien Search Request
    or the
    Vehicle Registration Application Request
    and mail it along with a $15 fee to:

    How long does it take to get a replacement title in New Jersey?

    If you request your New Jersey replacement title in person, you may receive the document the same day unless it has to be sent to a lienholder. If the replacement title must be processed by mail, the process will take 8 to 12 weeks.
